Emerging Demand in Pediatric Dentistry for Office-Based General Anesthesia by Dentist Anesthesiologists in the United States-Juniper Publishers

Juniper Publishers - Journal of Anesthesia Abstract Pediatric dentists have traditionally relied upon self-administered sedation techniques to provide office-based sedation. The use of dentist anesthesiologists to provide office-based anesthesia is an emerging trend. Recent research have examined and compared these two models of office-based anesthesia services a survey evaluating office-based sedation of diplomates of the American Board of Pediatric Dentistry (ABPD) found that over 70% of board-certified US pediatric dentists use some form of sedation or anesthesia in their offices. Furthermore, less than 20% administer IV sedation and 20 to 40% use a dentist anesthesiologist. Juniper Publishers - Journal of Anesthesia Abstract Purpose: Oral midazolam (MID) sedation is often used for pediatric dental procedures, however the quality of procedural sedation is variable and maybe inadequate for successful procedure completion. Intranasal (IN) sedative administration can be a useful adjunct as it has a more rapid onset and can be repeated as needed. MID can also be given IN however it burns and often results in crying. Routinely we use adjunct IN sufentanil, but due to a prolonged drug shortage and unavailability (> 1 year) we decided to assess the efficacy and safety of IN remifentanil (REMI), an ultra short acting synthetic opiate. There are only a few papers on IN REMI use. One report of 150 children given IN REMI as an adjunct to intubation did not find any problems and the kinetics appeared to demonstrate a fast onset of IN REMI and a rapid elimination as expected. Juniper Publishers - Journal of Anesthesia Abstract Study objective: The anesthetic management of a patient with MMD presents a challenge to the anesthesiologist as it may affect the prevalence of further complications of the disease. We hypothesize that anesthetic management significantly affects the long term neurological outcome in patients with MMD and an ideal anesthetic will reduce the length of stay. Study design: Retrospective cohort study Patients: We performed a retrospective review of 32 patients with MMD who underwent craniotomy and surgical revascularization. The data was divided into two equal case cohorts based on two equal time periods. Measurements: Data was collected on usage of anesthetic agents such as fentanyl, propofol, remifentanil, isoflurane, sevoflurane and desflurane. Obesity affects the quality and duration of life significantly [ 1 ]. It is increasing rapidly especially in young people and many chronic illnesses are developing at earlier ages. The socioeconomic burden is also increasing associated with it.Sustainable diet has a very important role in the treatment of obesity, but it seems nearly impossible to do it with severe obesity. At this point, surgical treatment is an appropriate and effective option for morbid obesity. Obese patients are a patient group which are difficult to manage due to anesthesia because of the lack of consensus on increased co-morbidity, difficult intubation and drug distribution. People with body mass index (BMI)>40kg/ m2 are defined as morbid obesity and BMI>60 are defined as super obese [ 2 ].

Anaesthetic Recommendations for Stiff Person Syndrome-Juniper Publishers

Juniper Publishers - Journal of Anesthesia Abstract Background: Stiff person syndrome (SPS) is a neurological condition with serious implications during anaesthesia if overlooked. Objective: Our purpose was to highlight the issues encountered during anaesthesia in patients with SPS and to evaluate the most appropriate anaesthetic management during surgery. Methods: A structured search was performed throughout various databases such as Ovid medline, Pubmed, Embase and Cochrane Collaboration for articles published from 1950s and after. The search included only humans and those who underwent procedures with anaesthesia. Results: Only 13 of such case studies were found due to the rarity of the condition. Amongst these 13 patients, most were in the middle to elderly aged groups and they underwent different procedures. Their response to different drugs used for induction, neuromuscular blockade or maintenance of anaesthesia also varied with some leading to life-thr
