Bleomycin Lung Injury: A Case Report and Review of the Literature-Juniper Publishers

Juniper Publishers - Journal of Anesthesia Abstract Bleomycin is a drug used to in the treatment of Hodgkin's Lymphoma, amongst other tumours. Bleomycin has been implicated in causing pneumonitis and pulmonary fibrosis, now termed Bleomycin Lung Injury (BLI). BLI is a much debated subject, in terms of both its pathogenesis and management of patients after diagnosis. In this paper, we reviewed the current knowledge of Bleomycin-Induced Lung Injury from a case report perspective. We will describe the case of a patient with BLI presenting to our ICU and discuss the available literature. Labetalol Hypotensive Anesthetic Protocol Paves the Way to Safe Open Abdominal Myomectomy-Juniper Publishers

Juniper Publishers - Journal of Anesthesia Abstract Objectives: Evaluating effect of oral labetalol pre-medication followed by low-dose intraoperative (IO) labetalol infusion on IO heart rate (HR) and mean arterial pressure (MAP), blood loss and need for transfusion and urine output (UOP) of patients undergoing open abdominal myomectomy. Patient & methods: Eighty-eight patients were randomly divided into: Study patients received 200 mg oral labetalol 2-hr before surgery and IO labetalol infusion (0.2mg/kg/hr) till completion of myomectomy. Control patients received placebo infusion and all patients received the same anesthetic protocol. Results: Operative blood loss and transfusion needs were significantly lower, but UOP was significantly higher in study than control patients. Insulinoma-Anaesthetic Implications with Review of Literature-Juniper Publishers

Juniper Publishers - Journal of Anesthesia Abstract Insulinomas are rare tumours that present with recurrent hypoglycaemic episodes. They are often misdiagnosed or diagnosis is often delayed due to their bizarre clinical presentation. Diagnosis of insulinoma is made based on history (Whipple's Triad), biochemical tests and imaging modalities. Treatment includes medical and surgical management. We intend to describe the anaesthetic management of three cases of insulinoma observed over a period of one year and review the existing literature. Though rare, when they do occur, these tumours pose a great challenge to anaesthesiologists. Vigilant monitoring of blood sugars intraoperatively is a must to counteract these wide swings, hence providing a better patient outcome.
